Immunization Information

 

All preschool children between the ages of four and six need a booster dose of two vaccines:

 

• Diphtheria, Pertussis, Tetanus and Polio (4-in-1 vaccine). Your child should have already received the 5-in-1 vaccine at ages two, four, six and 18 months.
• Measles, Mumps and Rubella and Varicella (MMRV) vaccine. Your child should have received this vaccine at one year of age.

 

Please make an appointment for your child to receive these important vaccines by contacting your local Public Health Office at the phone number noted below.  If your child has already received the above immunizations, please call the Public Health Nurse so we can update your child’s immunization record.
